Hospital Team Check-In Volunteer
You’ll be supporting our Hospitals teams by making check-in calls to carers of recently discharged patients. You’ll help them to feel more supported in their caring role by checking that the transition back home has gone smoothly. Your calls will help to improve their access to information and increase their awareness of additional support available in their community.
What you’ll be doing:
- Making check-in calls to family carers of recently discharged patients. Checking that their discharge went smoothly and that they have the support they need.
- Providing emotional support to family carers.
- Referring to the hospitals team any concern or change in circumstances or to request additional support from Suffolk Family Carers.
- Adding details of all calls to the Suffolk Family Carers database.
- Complying with our safeguarding policy and reporting any issues as appropriate.
From our offices in Claydon or your own home.

Hospital Team
This role suits you if you are: Patient, kind and tactful with a genuine interest in other people. You have good listening skills and an ability to empathise. You’re reliable, non-judgmental, realistic, and are able to cope with emotionally challenging conversations. You understand the need for confidentiality and commit to maintaining it. Previous experience with family carers is not essential; a desire to engage with family carers and understand the issues they face can be just as important.

- Be provided with the necessary induction, training, equipment and reasonable adjustments to do this role well and safely
- Be offered expenses to cover agreed travel costs between home and place of volunteering.
- Regular support and supervision will be given.
All volunteers will be expected to respect and uphold Suffolk Family Carers values and policies whilst volunteering with us. Suffolk Family Carers are committed to ensuring all individuals are aware of their role in safeguarding and appropriate training will be provided and must be completed.
