Carer's Assessments
If you are looking after someone you are entitled to a Carer’s Assessment from Suffolk County Council. You can have one regardless of the hours you spend caring each week, your financial circumstances or whether the person you care for receives any other support.
There 4 different statutory assessments Young Carer Assessment , Young Carer Transition to Adult Assessment; Adult Carers Assessment & Parent Carer Assessment
A Carer’s Assessment is an opportunity for you to have a conversation about your caring role and what you want to achieve in your daily life.
Your assessment may explore:-
- Do you still have time to do things you enjoy?
- Is caring impacting your health and wellbeing?
- Does your caring role affect your relationships with others?
- Is caring impacting your education, employment or volunteer opportunities?
Assessments may be carried out over the phone, face to face or via video call, and will be undertaken at a time and place that is convenient for you.(for Adults caring for an Adult a Self- Assessment on-line option is available)
As a trusted provider Suffolk Family Carers may carry out your assessment on behalf of Suffolk County Council.
Adult Family Carers
A Carers assessment is an excellent opportunity to focus on you as the family carer. The assessment will explore in depth what this means to you ,whether caring affects your physical, mental or emotional health, what existing strengths and support you have that could be developed, what information and support you might need to help you in your caring role. Adult Carers supporting an Adult now have an option to complete an online carers supported self-assessment – Request for Support. this link sends you to Suffolk County Council page to enable you to complete. The self-assessment is made up of different parts, firstly you have the option to complete a wellbeing question where details are gained to ensure you are given helpful information , the next part explores your caring role. After completing the carers assessment, Suffolk County Council will process this accordingly. Sometimes further support is required and Adult Social Care or a Trusted Provider will contact you in due course to have a conversation to see what practical or emotional support is available to help you.
If the self assessment option is not right for you , you can still request a full carers assessment be undertaken by an adviser, or you can ring customer first on 0808 800 4005
If the person you care for lives outside of Suffolk you will need to request an assessment from the local authority covering the area they reside.
Parent Carers
Parent carers, like all carers, are entitled to a Carer’s Assessment to find out what help they need and how these needs can be met. All Parent Carer assessment are completed by Suffolk County Council.

Help you could get
Help is available from a practitioner who can discuss with you your needs in your caring role, how it affects your wellbeing, your feelings, your choices about caring, and what help you need to support you as a carer of a child or young person. They will also help you find out if further help and support may be available to you.
The practitioner will work collaboratively with you to develop a plan around your needs. The plan is likely to include things that your family and friends can do to support you, as well as facilitating access to relevant local support services.
Young Carers
Suffolk Family Carers complete Young Carer Assessment’s and Young Carer Transition (to adulthood) Assessments on behalf of Suffolk County Council.
Requesting an Assessment
You can request a Young Carers or Transition Assessment by registering with us . If you are already registered with Suffolk Family Carers as a young carer you can request an assessment directly from your Adviser when you see them next or by contacting our information hub
Help you could get
The outcome of each assessment is different and unique to each individual. The wishes, feelings and aspirations of the young person are at the centre of the assessment conversation with the Adviser. Together they identify any impact of the caring role and to agree next steps of support. This may include, wellbeing guidance and support, face to face or online peer groups or condition specific information.
