Young Carer & Young Adult Carer Emergency Action Plan

Being a young carer can be difficult at the best of times, but it can be extra stressful if something unexpected happens, either to yourself or the person you are caring for. Planning ahead can help you manage these changes by being prepared, knowing what to do and who to ask for help and support.

The information on the downloadable young person Emergency Action Plan can be added to or removed as relevant to the child or young person’s age and situation.

It is key that the form is completed with the child or young person so they fully understand and agree to the decisions detailed.

For older carers we advise saving a copy of the completed document to their phone so they have it readily available.

For younger carers, we encourage the plan to be displayed in a visible place within the home so the child knows where it is and can reference it if needed. We encourage the child to decorate their plan with favourite stickers/pictures to remove some of the formality of the plan.

Where appropriate, share the plan with trusted adults and professionals supporting the family. This is important when detailing other people on the plan so they are aware of their role in the support of the family.

Revisit and review the plan regularly as details may change. For example, are contact numbers still accurate?
