My 84 year old husband is due to be discharged home from hospital with a short-term package of care, however I don’t know how I will be able to manage as he is still very confused. I have my own health problems and am worried how we are going to cope.
Thank you for contacting us, it is not uncommon to feel anxious in your circumstances. It is important when discussing discharge that you raise your concerns with ward staff, including your own health issues. Our hospital advisers, we can help to support you during any discharge discussion or meetings.
Once settled back home in a familiar environment it is not uncommon for a confusion to improve. A review will take place to see how you are both coping. Meanwhile, the carers will be assessing on each visit and it is important you share any concerns with the Team Lead.
Our adviser will be happy to call you after discharge to see how you are managing. Where appropriate, we can tell you about the benefits of having a Carers assessment, Family Carer Emergency Plan and any other services. Suffolk Family Carers can offer or any other organisations available to support you.
Suffolk Family Carers have dedicated Family Carer Advisers based at all three Suffolk’s major Hospitals – Ipswich, James Paget and West Suffolk. They can also support family carers of those in the smaller community hospitals within Suffolk. They provide support and guidance throughout a hospital admission as well as through the discharge process.
