Your carers assessment is an opportunity to discuss your caring role and the impact this may have on your day-to-day life. Through a conversation with an adviser we will look to identify any needs and work with you to find a practical solution to support you in your caring role.

Your Carers Assessment

Ahead of your Carers Assessment, you may find it useful to give yourself some time to think about your caring role, the impact that it has on you, and anything that might support you. Please see below what we will discuss during your carers assessment. This conversation usually takes around an hour

Your eligibility outcomes;

Does your caring role impact your ability to access the community for either hobbies, work or appointments?
Does your caring role impact your ability to maintain your home and/or garden?
Does your caring role impact your ability to eat well?
Does your caring role impact your ability to care for any other dependents?
Does your caring role impact your wellbeing or physical health?

Your life now;

Who you care for?
What necessary care and and/or mental wellbeing support you provide?
Do you share your caring role with anyone? What support network do you have available?
Does your caring role impact your physical or mental wellbeing?

The future;

Do you have any worries about the future?
What care are you able and willing to continue to provide?

Required Nationality and Immigration Documents

Suffolk County Council are required by law to evidence your nationality and immigration status. This information is used to help determine your eligibility for services. Suffolk Family Carers have been tasked by Suffolk County Council to collect this information on their behalf as part of the Carers Assessment.

You can do this by providing Suffolk Family Carers with a copy of any one of the acceptable documents that confirm your nationality. If you are not a British National you will also need to provide the relevant information that confirms your immigration status.

Acceptable Documents

Passport – front cover, details page, and any pages which identify immigration status (an expired British Passport is accepted)
Birth certificate – This is only acceptable for people born in the UK before 1 January 1983. People born in the UK on, or after this date are NOT automatically British. A birth certificate of someone born outside the UK is not evidence of nationality.
Full UK driving licence photocard – This is only acceptable for people born in the UK before 1 January 1983. People born in the UK on, or after this date are NOT automatically British..
Identity Card
Biometric Residence Card
Biometric Immigration Document – Biometric Residence Permit
Residence Card – includes an Accession Residence Card or a Derivative Residence Card
Current Immigration Status Document – must contain a photograph issued by the Home Office
Registration Certificate or Document Certifying Permanent Residence – These are issued by the Home Office to a national of a European Economic Area country or Switzerland.
Permanent Residence Card – These are issued by the Home Office to the family member of a national a European Economic Area country or Switzerland.

If you have settled or pre-settled status under the EU Settlement Scheme, please log into your account and provide a share code along with your date of birth.

Please note: If the document you are using does not show your current name, we will also need to see evidence of your change of name. For example if you use a birth certificate but have since changed your name, we will also need to see the relevant marriage or civil partnership certificate or deed poll.

Suffolk County Council or Suffolk Family Carers, if acting on their behalf, will need to see the original evidence when you are next visited in person. 

Sending your documents

You can send us copies of your documents using our secure upload facility or via post to our registered address. We encourage you to use one of these methods to protect your identity documents.

We ask that you include your unique ID provided in your Carers Assessment Welcome email in the filename of your document(s)

Please be assured that the information you provide is transmitted and held securely by us, and used solely to validate your nationality and immigration status, as required by law.

Verification of this request

Should you wish to verify that this information is required to complete your Carers Assessment you can contact the Suffolk Family Carers Information Hub. You may also contact Suffolk County Council via Customer First on 0808 800 4005 to confirm that they have requested Suffolk Family Carers to obtain this evidence

Information Security

The documents you provide will be held securely by Suffolk County Council and Suffolk Family Carers. Suffolk Family Carers do not retain a copy of your documents once they have been successfully passed to Suffolk County Council.

Suffolk County Council may keep its records for up to six years after a case is closed.  You have several rights regarding your personal data  and more information can be found on the Suffolk County Council website  Alternatively you can contact them via Customer First on 0808 800 4005.
