The Connected Communities Team has been working hard to get out and about in our communities over the past few months. This has been a challenge with so many restrictions in place, and of course we have done our utmost to ensure we are working safely. Our priority has been to let the people of East Suffolk know we are here and ready to support them if they need us.
For those of us who don’t know, Connected Communities is a social prescribing project here at Suffolk Family Carers. We work with anyone over the age of 65 who lives in Rural East and West Waveney, Wickham Market, Framlingham and surrounding villages, Woodbridge and Deben. As Community Connectors we will spend time getting to know the individual and find out what matters to them, and what changes they would like to make to improve their wellbeing.
We then work with them to come up with a plan and link them to other people or services in their community who can help. We are primarily here to help those who may be feeling lonely or isolated, but we can also help with a range of issues such as long term health conditions, low mood, money problems, bereavement, housing issues and more.
This year has been hard for so many, and as we go into 2021 we want to let people know that we are here and we can make a difference together. We have spent some time trying to get to know those businesses and services in our communities who may come across people who need our help, and asking them to help us spread the word.
As we go forward, we are asking locals to help us make ‘Time for a Yarn’. If you live and work in any of our communities, we would be really grateful if you could have a yarn with those you live or work with about Connected Communities and the work we do. We can provide you with all the information leaflets and posters to provide accurate information to those in your community who we may be able to support.
