Respite On Prescription Feedback
According to our records you have received funding from us in the past 15 months as part of our Respite on Prescription Service.
The Respite on Prescription (ROP) service helped enable you to have a medical procedure/treatment by providing funds to support you with replacement care for the person you care for. To help us obtain funding to continue this service in future years we are now asking for your feedback on your experience with using ROP.
In return, once you submit your feedback you will be provided with an opportunity to enter our quarterly prize draw for a chance to win a £25 Sainsbury’s voucher should you wish to do so.
