I am sole carer for my partner and I worry what would happen if I was, say, unexpectedly unable to look after them?
There is something called a Family Carers Emergency Plan, which we can help you complete, or here is a link for you can complete yourself.
In this plan you set out what needs to happen for the person you care for in the case of an emergency, important information, like their care and support needs, relevant information about your home, and GP details. You identify people who are able to support the person you care for in the case of an emergency. If you don’t have anyone, then it is important that this is recorded too. The plan will then be held by Suffolk County Council. You will receive a card/key fob with a unique reference number.
In an emergency, either you or someone you ask, needs to give your ID number to Customer First, (gateway to Suffolk County Council, Social Care) 0808 800 4005 who will activate the plan, they are available 24 hours a day and there is an option on the phone to press for someone being at risk, so the call is answered as quickly as possible. They will then activate the plan, call your chosen contacts and ensure care needs are met.
